Ventilator Home Management

Arcadian Healthcare is a leading provider of home ventilator management.  We specialize in assisting hospitals to facilitate the discharge of ventilator dependent patients into the homecare setting with ease (adult and pediatric).  We take a multi-disciplinary approach coordinating with case management to ensure the safest admission to home with outstanding long-term outcomes.

Since 1995, we have discharged over 250 patients into our clinical management program.

The Goal

The goal of our Ventilator Management Program is to improve the quality of life for the ventilator dependent patient and their family while reducing healthcare resources and expenditures. This is achieved by providing a safe and cost effective discharge plan. Through education and training to all caregivers, we can minimize the potential of hospital readmissions due to a respiratory related condition. We have implemented critical pathways that must be followed to ensure the safety and success of the patient at home.

Approach

Our program is based on an inter-disciplinary approach. In order to successfully manage a ventilator dependent patient at home we need to establish a solid support group. This usually includes the physician, case managers, caregivers, Respiratory Therapists, Nurses, and additional health care professionals as needed.  Physician’s orders and Joint Commission on Accreditation of Homecare  Organizations (JCAHO) standards mandate our program.

Training

Since education is a vital component of this program, prior to discharge we educate all involved caregivers on the equipment, supplies and necessary home care techniques and offer follow-up training to assure continuity in care. Due to our broad scope of service, we can provide all necessary medical equipment and supplies required for successful home care. Our clinical staff is available 24 hours a day, seven days a week.

Weaning

Due to the clinical expertise of Arcadian’s clinical staff we have the ability to successfully wean patients off ventilator dependency and potentially from their tracheostomy. If every criteria of our program is met, we anticipate positive homecare outcomes. Our clients who meet our weaning criteria have a 93% success rate, which is an outstanding achievement. 

Outcomes

We monitor compliance regularly in order to monitor our patient’s progress at home and offer additional clinical services and education when necessary.

Since 2000, less than 3% of our patients required hospital re-admission or physician office visit due to an exacerbation of the patients respiratory status. This is a direct result of ongoing clinical assessments to the plan of care, ensuring caregiver compliance skills, and to the physicians treatment plan.


COPD Management and Compliance

Arcadian Healthcare has recently developed a new and innovative disease management program that assists physician’s in the home-management of patients diagnosed with Chronic Obstructive Pulmonary Disease (COPD), Asthma or other breathing disorders.

Scheduled home visits by our clinical staff will:

  • Ensure optimal equipment performance and patient compliance of prescribed oxygen and respiratory care therapies.

  • Promote desired clinical outcomes through on-going assessment of patients to their airway management treatment plans and proper oral inhalation treatment procedures.

  • Measure clinical and physiological outcomes as part of the MD Treatment Plan to facilitate physician intervention as needed.

  • Improve compliance to Medication Orders, Oxygen Therapy Rx, reinstruction as needed to breathing techniques / Rehab Program Plan of care.

 Arcadian Clinical Advantage

1. Assisting in the multi-disciplinary and continuum of-care at home:

  • Evaluate each patients individual oxygen system and clinical respiratory equipment needs ensuring efficacy of therapy.

  • Patient/caregiver clinical respiratory and durable medical equipment (DME) training at facility and at home.

2. Home safety evaluation for ambulatory oxygen and respiratory equipment use.

3. On-going practitioner clinical home care visits as determined from plan-of-care.

4. Written/Verbal communications to medical staff documenting patient's clinical status.

Respiratory Clinical Assessment may include:

  • General status/complaints, pain management, infections, weight gain/loss, nutrition.

  • Auscultation and vital signs

  • Capnography or Pulse Oximetry  (rest/ambulatory)

  • Spirometry and Peak Flow

  • Homecare assessments submitted to rehab facility and physician with phone contact if timely intervention is needed.

This program is especially beneficial for those patients who have entered into a pulmonary rehabilitation program.
